UQHC is governed by a number of Policies determined by Hockey Australia, Hockey QLD, BWHA, BHA and the University of Queensland.

BLUE CARD REQUIREMENT AND APPLICATION

Any member of the Executive can sign this form for you.

UQHC requires all Executive members, Coaches and Managers working with children under 18 to hold a Blue Card. If you are exempt (Doctor, teacher etc) you will still be required to provide us with an exemption certificate in order for us to ensure we meet our responsibility under the QLD and HQ Child Protection Policy.
